To be eligible to receive an Accommodation Plan a student must meet the following requirements:
- The student must have been evaluated by a professional with the appropriate credentials to diagnose the condition (e.g. psychologist, diagnostician, neuropsychologists, or physician).
- A full report must be submitted to the Learning Specialist.
- The report must include:
A clearly stated diagnosis of a disabling condition, as listed in the current edition of the Diagnostic and Statistical Manual of Mental Disorder (DSM) and/or meet the criteria established by the New Mexico State Department of Special Education. - Data in the form of standardized test scores on global measures including but not limited to: a measure of cognitive ability, academic achievement, measures of information processing, health and family history, early education, native language and language proficiency and discussion of any conditions that might ordinarily rule-out a diagnosis such as lack of educational opportunity.
- The evaluator must clearly state how any recommended accommodations are connected to the diagnosed disability, e.g., if extended time on tests is recommended, the clinician must explain how the test results indicate that the student’s ability to take timed tests is impaired.
- The evaluation must have been conducted within the last five years.
- An AD/HD diagnosis is not valid if determined solely by a physician. Please see #5 above for valid testing requirements.
